I'm 31 years old and ready to get into scuba after years of just snorkeling around. I had a tympanoplasty and mastoidectomy when I was 18 years old. No problems equalizing and no perforated eardrums. Am I likely to be disqualified by my doctor for scuba, or is it a case by case thing?

Yes, it is a case by case basis. I would go to a doctor that is versed in diving medicine and conditions. DAN-Divers Alert Network is a good place to start. I didn't find any articles directly dealing with tympanoplasty or Mastoidectomy except to say that you should wait at least 2 months to dive after tympanoplasty to ensure it's healed.
