It's really easy and cheap. Get a couple of feet worth of 1/8 or 3/16 bungee cord from REI ($0.15-0.30 a foot), and a ziptie. Measure to length, a knot at each end (perhaps with a dab of crazy glue on the knot to keep it from unraveling) and ziptie it behind the regulator mouthpiece.

The bungee is better than the pre-fabbed ones or the surgical tubing. The prefab ones are too lose at the mouth piece IMO. I bought one and threw it in my save a dive.
The way you make them with surgical tubing to take a length (whatever is comfortable) lap the ends. Use 2 zip ties so you end up with what looks like a pair of lips and slip your mouth piece between the lips. With the bungee you kind of do the same to make the lips, but instead of zip ties, you make two knots. The knots will slip a little to tighten up the necklace around the mouth piece. ![]() Here is Gombessa's method. ![]()

Easy as pie, I used some thin bunge that I had laying around and on each end I tied a knot. Then I attached with a zip tie, final cost: free!!

Here's a link to a similar thread that shows examples.
DIY reg necklace? I have the bungie with the slip knots like Splitlip explained, and like the photo's shown by Compudude in the above thread. There are pros & cons to using the knots vrs the zip ties. With zip ties, you don't have to worry about the octo being pulled out. But, if someone for your octo rather than your primary, they'll be jerking on your neck trying to get the octo. If you make the loop tight around the mouthpiece, there is very little chance that the octo will slip out on it's own. I have yet to have it come out unintentionally (knock on wood). I don't profess to know which 1 is better, but that's what works for me.
