I guess I don't get why men don't wash, it's gross not to. But then you still have to grab the door handle to get out of a public restroom, rather makes the whole exercise a waste of time.

That's what the paper towels are for on the way out the room. No matter what, I make sure I grab one to grab the handle with, because there are people that just don't wash, no matter what....

If it's a sit down visit, then there's a manditory hand washing.

If it's a stand up, there's no need to. Personally, I learned not to pee on myself a number of years ago, but splashback does nullify the no-wash rule.
