here are my "stock" photos of a 40 run under my arm
and a "halo"

It was a time before insurance liabilities, lawsuits or beauracratic regulation of the "sport". Guerrilla divers didn't need "Buoyancy Compensator Vests". In fact, "Anyone who needs a BC deserves to drown" was a popular adage. Exploration and the Hunt came first, excitement and fun followed. Splitlip, thanks for the photos. I will definitely work with the hose under the right arm more. That's the fun of the sport, eh? Trying new things.
The octo really doesn't feel awkward. I have it under my arm for streamlining, but may take your suggestion to goto a 22-inch over the shoulder. Even under the arm, the octo was comfortable to breathe from underwater. This was the first time I used the octo on a necklace; once, again, just to see if it's something I like. I was concerned it would get in the way; it didn't, so I think I'll stick with it. Thus, the 22-inch hose is in the future. It's insightful observations from experience divers, such as yours, that I look for on this board.

Having not ever played touchy-feelie with a Miflex hose, since where I live the only way to do that is to buy it first, can anyone tell me if the diameter is close enough to a standard hose that it can be held by a standard hose clip?

When I ran a 40" hose, I found it necessary to have something redirect the second stage. A 90 degree angle adapter would be a good option (no real chance of failure). If you're going to use a swivel, be warned, most are TERRIBLE. The only two on the market that I believe still haven't had a failure are the Atomic swivel (very expensive) and the Kirby Morgan swivel (less expensive, but still pricey). If you're going the swivel route, I'd suggest sticking with one of these. Otherwise, consider the fixed adapter.

That is an Atomic swivel. If I were to do it over again, I would go with the Dive Rite 90.
