This past Saturday, I did a Dumb@$$.
Our dive club did an underwater poker tournament for our fun dive at Athens (max depth, 20 feet). My buddy and I found our poker hand quickly (full house, 5's over 9's), then we lost each other in the muck that is Athens. No problem. I look at my computer on my wrist to determine depth and bottom time to plan ascent. No computer. Oooops... Since I was dealing with a Lost Buddy situation, no computer, less than 15 minutes dive time, only 20 feet of depth: I ascended at standard 30 feet per minute (one kick cycle every 2 seconds for me) and no additional Safety Stop (the entire dive was a Safety Stop). Buddy popped up 30 seconds later about 4 feet from me. (Yes, viz is that bad). What piece of gear have you forgotten after you splashed?

Embarrasingly...my fins. Yes, my fins! One very early dive ( I was 15) I had gone down to Beaver Lake, did a few shore dives with the plan of going to the foundations later in the day via boat. So, after getting to Lost Bridge marina, we ltook the gear out of the car and put it on the boat. only when we got to the foundations, I realized that my fins didn't get on the boat. Tried to dive without them-didn't work. Since that dive, I have always been sure to not only double check for my fins, but carried a spare pair just in case.
BTW, I haven't forgotten them since.

Bonaire, April 2008, first dive of trip. Got in the water, buckle on fin broke. Unzipped wetsuit, it's hot! Waddled my soggy butt into the shop for replacement, as good of a time as any try out those spring straps!!! Apply new straps, put gear back on, go for dive, feel kinda cool and wetsuit is ill fitting. Get out, take off BC, unzip wetsuit... OH WAIT, it's already unzipped, DOH!!!!
