Here is the information posted on the Deco stop.

It is with great sadness that we want to share the passing of Phil Patz.
Phil died on Thurs. 25 June while diving on the Tennie & Laura. Phil and Bill Prince were in the team first diving this wreck several years ago. Phil's great laugh, his love of all things outdoors and his wry sense of humour will be greatly missed by all those who knew and loved him.

Details of a service will be posted at a later date. Condolences can be sent to Phil's mother at:

This is something like the fourth rebreather death over the past several weeks. All very sad and concerning.
With all due respect to the departed. I wish that after these tragedy more accurate and specific cause would be available.
As a rebreather diver I would rather learn from these tragedies then end up repeating the causes.

What information do you have that we don't? Just curious how you came to the conclusion that he was using a rebreather. Nothing implied here, just wondering.
